What are the requisites, or does it differ from state to state, parish to parish?

The requisites are:

1. That you have never been Catholic before, sounds dumb, but it is a totally different game if you are fallen away.

2. You go through proper catechesis. For most parishes this is RCIA, but that is not a requirement. Personal instruction works well.

3. Proof of Baptism or knowledge of not being baptized.

The rules don't differ from place to place. Proof that one is not Catholic. Proper Catechesis and proof of baptism/knowledge of not being baptized.

That is all. Pretty simple. The Catechesis isn't simple, but that goes without saying.
