It’s pretty managed at the family Catholic church here, but I am wondering about other parishes precautions for high risk visitors and am wondering how common this is.

Well, the parish I attend dropped social distancing and all that probably 22 months ago or so. Things have been fine since.
The parish I work at, however, tends to be quite a bit older; as yet, we're also effectively back-to-normal. If we host a funeral or wedding and the guests themselves wear masks, we'll wear masks too. If not, we won't. I've seen perhaps 5% of parishioners mask themselves this year, and perhaps 4 students out of ~325 do so as well.

As for how homebound ministry works, I simply don't know. Our priests are human, though, and I'm sure they make whatever judgment call seems most fitting.

We haven't had anybody who's high-risk ask for accommodation (as far as I'm aware) but I'm sure we'd take whatever reasonable steps they wanted.
