if someone is struggling with an addiction of something that is otherwise a mortal sin, they are not freely choosing the action.

is the addiction a mortal sin in itself?
is someone with an addiction of an otherwise mortal sin forbidden from receiving communion?

Addiction lessens one's culpability so that something that may be grave matter may perhaps not be a mortal sin.

As far as receiving Holy Communion, I have heard of instances in which a priest has allowed a person in this situation to receive.

It is really something that must be dealt with on the level of the particular. I would strongly suggest that a person in this situation get with a wise and capable spiritual director and/or confessor. This priest could help the person overcome this addiction as well as discern the matters you asked about.

God bless.
