Typically, a Benedictine priest gives the medal a blessing which gives it the graces proper to exorcism - not just the head-spinnin', pea-soup-spittin' type, but also the types of exorcism we need every day in order to keep the Evil One at bay. This type of blessing can only be given by a Benedictine priest; however, any priest can give it a blessing, making it a sacramental. Hope this helps.
